Enrolling at Ilam Medical

Ilam Medical Centre welcomes new patients from inside our enrolment zone.

PLEASE READ THE FOLLOWING INFORMATION

Patients need to consider carefully if they require any repeat medications around the time of enrolment as the process can take 3-4 weeks. Until your notes are received from your previous practice, and you have had your first appointment, your GP will not be able to issue any repeat medications. It is the patients’ responsibility to manage their requirements over this transition period.

We require notes from previous practice to be transferred to ensure your new Doctor has a full and clear medical history.  If patients do not agree to transfer notes from their previous practice, we will not agree to enrolment.

All new patients are required to have a double appointment with their new GP for the first visit.  Unfortunately, we have had a number of patients who attend for this appointment and leave without paying.  We now require patients to pay for their first appointment prior to us requesting their notes from their previous practice.  We will not progress your enrolment process until payment is received. This means that you will remain enrolled at your previous practice until prepayment of your first, double appointment is received

 

Our enrolment process and requirements are listed below:

 

  1. Patient completes Enrolment form, Patient Clinical Update form & Terms of Trade.
  2. All three forms need to be presented at the counter with Original ID for checking
  3. IMC staff will check documentation and put in queue for loading onto our system
  4. Patient details are loaded into our system. Patient is emailed requesting payment of a double appointment fee.
  5. Once payment fee is received, patient will be enrolled on the National Enrolment System and notes requested from your previous practice. Patient is advised that notes have been requested and to allow 10 days before calling to make an appointment.
  6. Once notes are received from your previous practice, a double appointment can to be made with GP.

  2. Are you living within our Enrolment Zone, see map
  3. Read our Terms of trade
  4. Download and complete the followingTerms of Trade,
    Enrolment form & New patient Health questionnaire
  •  Please note that every patient over the age of 16 is required to sign their own enrolment form.
  •  Bring completed forms along with photographic ID into the practice. Please do not email completed forms as original ID must be sighted by our staff in the practice. You will also need to bring in evidence of your current address to confirm you are living within our enrolment zone.  Power, rates or telephone account etc.
  • We will only accept patients who consent to have their patient file transferred from their previous practice due to medico legal risks.  This includes patients who have moved to New Zealand.  We would require the notes to be translated into English and for that to be received prior to enrolment confirmation.
  •  NZ citizens can bring one type of photographic ID eg, Drivers licence, Student ID, 18+ card or passport.

  • It is important to note that you will not be able to have an appointment or obtain a repeat prescription at this clinic until all your enrolment and identification information has been processed, prepayment of first appointment and your notes transferred from your previous practice.  The process to obtain notes can take 3-4 weeks.  Please keep this in mind as you may need to attend your current GP to ensure the continuation of your current medications (if any).
